Program Waiver

A separate waiver must be completed for each person attending (each child and each adult). Please submit this form once per participant. After submitting one waiver, refresh or return to this page to complete the waivers for additional participants. For participants planning to attend multiple programs, please submit one waiver and it will be kept on file for the calendar year. Additional waivers may be required for specific venues.

Please list any allergies (food, environmental, or other), medications, or medical conditions that may come up during a program. Include severity and any action we should take if needed.

Every learner is different. Let us know about any sensory sensitivities, communication preferences, mobility needs, or strategies that help your child feel comfortable and engaged. There is no detail too small.

This information is used only to support your child during our programs and is kept confidential. Families are responsible for updating Discoveries in the Desert of any changes to allergies, medical needs, support needs, etc.

Assumption of Risk & Release of Liability

Assumption of Risk

I understand that participation in Discoveries in the Desert LLC programs involves inherent risks, including but not limited to physical activity, outdoor environments, equipment use, and participation at third-party venues such as recreation facilities, museums, aquariums, zoos, activity centers, and other similar locations.


Third-Party Venue Participation

I understand and acknowledge that programs take place at independently owned and operated third-party venues. These facilities, including their equipment, activities, staff, and operations, are not owned, managed, or controlled by Discoveries in the Desert LLC.

Additional Terms and Policies

Indemnification

I agree to indemnify, defend, and hold harmless Discoveries in the Desert LLC from any and all claims, liabilities, damages, losses, or expenses, including reasonable attorney’s fees, arising out of or related to participation in the program or any breach of this agreement.


By submitting this form, I acknowledge that I have read and agree to all terms of the Participant Liability Waiver & Release, including the assumption of risk, release of liability, third-party venue terms, and indemnification provisions.
